I have experienced this and have done a little experimentation. Here's what I know happens on occasion...

Shutter trips as if picture was taken.
Picture does not appear on LCD immediately.
Picture has not been recorded to card.
Swapping or formatting card does not help (trouble is within camera).
It's happening on my XTi perhaps 1 in 40 shots.
